All-Terrain Wheelchair Reservation Form
The Forest Preserve District is happy to loan for free an aluminum, all-terrain, lightweight wheelchair that can be easily pushed over dirt, gravel, rocks, grass, snow, and mud for use at Fullersburg Woods Forest Preserve in Oak Brook. It is a manual wheelchair that rolls on three large, low-pressure tires.

Size: Medium, 16.5-inch-width seat, Carries up to 200 pounds
Reservations are for Fullersburg Woods only and are based on availability.
You can submit a request to reserve the all-terrain wheelchair one of four ways:
- Complete the online form below.
- Download a fillable reservation form in English (PDF) or Spanish (PDF), and bring the printed form to Forest Preserve District headquarters at 3S580 Naperville Road in Wheaton Monday – Friday 8 a.m. – 4 p.m.
- Fax the completed form to 630-933-7217.
- Mail the completed form to Visitor Services, Forest Preserve District of DuPage County, 3S580 Naperville Road, Wheaton, IL 60189.
